Rationale	- Without a wheelchair or toilets:wheelchair tag, accessibility of a poi or toilets of a poi is already unknown as as stated explicitly on toilets:wheelchair=*. - It was also a bug on wheelpmap.org that added the unknown status when a user did not fill in all the fields or change the previous value. This bug seems fixed because I haven't seen a recent case. EDIT: another bug was found. - In Iraq/Saudi Arabia, some are also due to a 7 years old undocumented import. - Some mappers also reported to me that they filled "all tags" with the unknown value because their editor proposed a lot of field and they didn't know what to put.

Rationale	the absence of the name tag in itself shows that the object does not yet have a name, so adding fixme to point to missing information is inadvisable, useless and even counter-productive since it hides the real fixme, especially as at least one editor does not (yet) remove the fixme when a name is added
