This tag indicates whether or not there is a ramp available. It's normally used on highway=steps. A ramp can, for example, be built next to a stairway in order to allow access to higher ground for people who use a wheelchair or have a bike.
A standalone ramp without steps can be tagged with highway=footway and incline=*.
In combination with ramp:portable=yes this tag indicates that an element (like a public transport platform) has a portable ramp.
Ramps
This is for ramps that are included in the stairway itself. Ramps that run physically separated (e.g. separated by a fence, height difference or wall), should be drawn separately.

ramp=separate | A ramp making the same journey exists, and is mapped as a separate OSM object. The separate object should be tagged with, e.g., highway=footway and incline=*. |
ramp=no | No ramp at all. This is the default, but it's useful to be definitive especially when routing for people in wheelchairs. |
Ramp types
A stairway can theoretically have several ramps of different types. This tagging scheme allows to specify which one exists at given location, allowing to distinguish cases where ramp only help cyclists from one usable by strollers or wheelchairs.

A ramp that can be used by wheelchairs (typically, also by strollers). Such a ramp should be no steeper than 4-6% and wide enough to accommodate a wheelchair. A stroller ramp might be steeper or narrower. |

ramp:portable=yes |
yes indicates the provision of a ramp deployable for users (typically wheelchairs) to traverse the feature. They can be found, for example, at railway platforms to help wheelchair users getting on and off trains. Further characteristics of a portable ramp can be described using the keys:

Presets
Ramp is included in the internal JOSM preset (since version 8011).
You can also add presets that uses the proposed scheme to JOSM. Just add the "Steps properties" preset in the JOSM presets properties (http://osmtools.de/josm/steps.xml)
